Step 2 — Fill out your HTMA Intake Form

Once your kit arrives, you’ll send in your hair sample —

but I also need your intake form to prepare your personalized analysis and support recommendations.

Your kit ships right away.

Your intake form simply helps me prepare your customized supplement recommendations, nutrition notes, and mineral interpretation while we wait for your lab results.

1. Your kit ships soon

You’ll receive tracking information as soon as it’s on the way.

2. You submit your intake form

This gives me insight into symptoms, stress patterns, lifestyle, and current support.

3. You collect your hair sample

A quick, simple process — instructions are in your kit.

4. You mail your sample to the lab

Your kit includes the envelope + mailing instructions.

Once the lab receives your sample, analysis begins automatically.

5. You’ll receive your results ~2 weeks after the lab receives your sample

When your results return, I’ll create:

— your personalized mineral analysis

— adrenal + thyroid pattern overview

— supplement guidance

— nutrition & lifestyle notes

— gentle foundation building

And everything will be delivered to your inbox.
